The transition from paediatric to adult care is a crucial phase, defined by the US Society for Adolescent Medicine as “the purposeful, planned movement of adolescents and young adults with chronic physical and medical conditions from child-centered to adult health-care systems”.1 It requires careful planning and thoughtful implementation to not jeopardise what has been achieved during paediatric medical care.
